Senior Deficient Chart Specialist, Medical Records

This Role:
Responsible for the processing, reconciling and communication of assigned clients as it pertains to Unable to Code charts.
Key Responsibilities:
Work independently to perform daily assignments\tasks and assist with additional QA assignments such as but not limited to: EMR searching, New Hire RAI reports, and LHI search results
Work closely with Team Lead and Supervisor performing tasks assigned to support the MR Deficiency daily workflow processes
Search proficiently among multitude of client Electronic Medical Record systems, as well as ensuring documentation makes it through to the end of the process
Send provider messages through both EMR and outlook email as well as monitor and work responses in a timely fashion
Provide evaluations and training of new and existing Medical Records staff as needed 6. Monitor and work LogixRequest client comments and updates daily
Prepare accurate RAI reports for the assigned clients and send them encrypted to the appropriate contacts
Audit processed documentation for accuracy using provided coding definitions and reconcile against the RAI reports. Ensure new documentation added to the charts results in a coded chart and if otherwise, raise the issue to management
Review receipt and work RAI responses within a 24-hour timeframe and notify management of any discrepancies or client concerns
Communicate with Physicians and other healthcare professionals to obtain additional information or answer questions via internal and external communication modules
Alert management of any issues with files such as changes in formats from the client or corrupt files sent to us, along with any other issues with files that may arise
Adhere to the Code of Conduct and be familiar with all compliance policies and procedures stored in LogixGarden relevant to this position
Report breaches, instances of noncompliance, client complaints, problems, or similar instances to Supervisor\Manager to protect patient HPI

Required:
High School Diploma or equivalent combination of education and experience required
Proficiency across the entire MS suite: specializing, but not exclusively, in Excel, Word, and Teams
Intermediate hardware and software knowledge to efficiently use programs and applications
Must be able to make suggestions on workflow improvements or system efficiency
Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedural manuals
Must be detail oriented and capable of navigating medical records proficiently across multiple platforms
Ability to work in a fast-paced environment at a high-volume level of accuracy
Ability to communicate effectively verbally and in writing
Ability to work independently and perform functions with minimal supervision
Ability to apply common sense understanding to carry out instructions
