About the Role
The Care Coordinator plays a central role in ensuring high‑quality, person‑centred care is delivered to clients across Oxford and the surrounding areas. This position requires a proactive, organised professional who can balance scheduling, compliance, communication, and crisis management with confidence and compassion.
You’ll be the operational heartbeat of the service — coordinating care packages, supporting care staff, liaising with families and professionals, and ensuring smooth day‑to‑day delivery of care.
Key Responsibilities
Develop and maintain effective rotas to ensure all care visits are covered in line with client needs and contractual requirements. Respond promptly to changes in care packages, staff availability, and emergency situations. Participate in the on‑call rota, providing out‑of‑hours support for urgent issues. Conduct client assessments, reviews, and risk assessments in line with regulatory standards. Maintain accurate, compliant records using digital care management systems.
