It takes a Village to care for our patients, and as a Care Coordinator (CC), you will be at the heart of all the action. Our CCs work together as a team to facilitate planning and organizing all critical activities for our patients. The patients we care for are some of the most complex and need an excellent care team to help manage their chronic conditions. The Care Coordinator works closely with Nurse Practitioners and RN Case Managers to ensure our patients receive high-quality care.
LOCATION:
- Commute to multiple DaVita Clinics and partner practices in the South Dayton, OH metro area.
- Maximum commute from/to centrally posted location is 90 minutes.
- Mileage reimbursed - MUST have reliable transportation.
Position Details:
- Work in clinics/partner practices 4 days per week, remote the remaining day(s).
- Full-Time Monday through Friday schedule.
- Start time between 7:00 and 8:00 AM - flexibility required.
Responsibilities:
- Complete non-clinical screenings and escalate as needed.
- Identify eligible resources for patients.
- Navigate resources for patients.
- Assist with referral process & coordinate provider appointments.
- Support the clinical team in moving patients through the continuum of care by communicating and completing tasks as directed.
- Implement and document discharge care plans, including referral forms, clinical info gathering, chart initiation, and appointment scheduling.
- Communicate with patients for scheduling requirements.
- Maintain accurate, complete documentation with proper signatures and confidentiality.
- Ensure timely document delivery without compromising privacy.
- Demonstrate clear and organized communication skills.
- Act according to DaVita’s mission and values to deliver high-quality care.
Qualifications:
- High school diploma or GED.
- At least two years of experience in a professional office preferred.
- Ability to handle confidential information with discretion.
- Excellent verbal communication skills.
- Proficiency with Microsoft Office/Excel/Outlook.
- Experience with Electronic Medical Records preferred.
- Commitment to service excellence and patient satisfaction.
- Ability to work independently.
Preferred Qualifications:
- 1-3 years clinical medical office experience.
- CNA, MA experience a plus.
- Experience coordinating patient care (referrals, ACO management).
- Experience navigating insurance payors.
- Experience in direct patient care/support.
- Experience in a medical office environment.

Salary/Wage Range:
$19.00-$27.00/hour.
Compensation depends on qualifications, skills, and experience. DaVita offers a comprehensive rewards package.
