Community Care Officer

Nottinghamshire County Council are further developing all-age ways of working in Social Work Community Teams and permanent Community Care Officer vacancies have arisen across our North, Mid and South districts of the county. We are also recruiting across our Urgent and Emergency Care Teams in all areas of the county. W we are looking for dynamic staff with relevant knowledge, skills and expertise in all areas of Social Work practice and hospital discharge work. The successful candidate will work as part of the Social Work Community Team or a hospital team within an all-age approach that fosters collaboration and innovation. We support people to live ordinary lives in the place they call home with the people and things they love. We start by understanding what a good life looks like for them and how we can work together to achieve it. We are recruiting Community Care Officers to work alongside people and their families seeking or drawing on support to live lives that they chose to lead, whatever their disability, age or stage in life. You will be responsible for identifying a person's goals and aspirations and the initiation and coordination of a range of outcomes to meet them such as the potential for reablement and enablement opportunities and referrals to support access to those services. Using strength-based approaches you will promote the principles of choice and control, personalisation, and self-directed support to ensure that people can assess and manage their own needs, risks, and uncertainties. You will work with partners such as health colleagues, other statutory and voluntary organisations as well as other Community Care Officers, Social Workers and Occupational Therapists. There is an expectation that Community Care Officers work flexibly across other districts to manage pressures when required.
